BY CLICKING “I AGREE,” CHECKING A RELATED BOX TO SIGNIFY YOUR ACCEPTANCE, USING ANY OTHER ACCEPTANCE PROTOCOL PRESENTED THROUGH THE SERVICE, OR OTHERWISE AFFIRMATIVELY ACCEPTING THIS CONSENT, YOU ACKNOWLEDGE THAT YOU HAVE READ, UNDERSTOOD, AND AGREED TO BE BOUND BY THIS CONSENT. PURPOSE
This Telehealth Consent (“Consent”) explains how healthcare services may be delivered to you via telehealth technologies and outlines your rights, risks, and responsibilities. NavioMD facilitates access to remote clinical care through its technology platform and a network of contracted partners and providers, including Qualiphy, The Pharmacy Hub, Steer Health, and licensed third-party laboratories.
TELEHEALTH SERVICES
Telehealth involves using electronic communication technologies to provide healthcare services when the provider and patient are not physically present in the same location. These services may include:
- Video or phone consultations
- Secure messaging and chat interfaces
- Digital intake and health assessments
- Prescription management and refill coordination
- Lab orders and result review
- Asynchronous clinical evaluation (where permitted by law)
Through the NavioMD platform, you may interact with:
- Licensed medical professionals via our telehealth partner Qualiphy
- Digital care tools and scheduling/reminder systems via Steer Health
- Pharmacy services and prescription fulfillment via The Pharmacy Hub
- Lab testing and diagnostics via certified third-party labs
Providers may assess your condition and, based on their judgment, determine the appropriateness of care and treatment via telehealth.
JURISDICTIONAL SAFEGUARDS & LICENSURE COMPLIANCE
NavioMD is committed to operating in full compliance with all federal and state laws regarding the practice of medicine, pharmacy fulfillment, and telehealth services.
- Pharmacy Fulfillment: The Pharmacy Hub is licensed to dispense and ship prescriptions to all 50 U.S. states.
- Telehealth Services: Our clinical partner, Qualiphy, provides services through licensed professionals and currently does not support telehealth care in Alabama or Mississippi.
To ensure licensure compliance:
- Patients are required to provide a valid state of residence and shipping address at the time of intake.
- Our platform dynamically restricts access to care, payment processing, and prescriptions in states where provider services are not currently available.
- Patients in unsupported states are blocked from proceeding and notified during onboarding.
- Our team conducts regular audits and oversight to maintain ongoing regulatory compliance.
If you are located in an unsupported jurisdiction, you will not be able to access clinical services via NavioMD until service becomes available in your area.

RISKS AND LIMITATIONS
As with any medical care, telehealth has potential risks, including but not limited to:
- Limited ability to perform physical examinations
- Misdiagnosis or missed diagnosis due to remote limitations
- Delays due to provider unavailability or technical failures
- Emotional discomfort during mental health sessions
- Data breaches despite security measures
- Incorrect or incomplete information impacting care
- Limitations in prescribing due to state laws or clinical discretion
- Inaccuracies or limitations in lab results
APPROPRIATENESS & EMERGENCY SITUATIONS
NavioMD and its partners are not emergency service providers. In an emergency, you must seek in-person care or call 911.

PROVIDER ROLES & PARTNER RELATIONSHIPS
Care and administrative support are facilitated through the following NavioMD partnerships:
- Qualiphy – Delivers clinical services via licensed physicians, nurse practitioners, and physician assistants.
- The Pharmacy Hub – Our pharmacy fulfillment partner, licensed in all 50 states, ships medications to your door discreetly.
- Steer Health – Provides engagement technology including intake, scheduling, reminders, and follow-up communications.
- Third-Party Laboratories – Conduct at-home or offsite diagnostic testing. Results are routed to your provider for review and clinical decision-making.
Providers act independently in clinical matters and determine whether telehealth is appropriate based on your medical condition, location, and treatment needs.
PATIENT RESPONSIBILITIES
As a patient, you agree to:
- Provide accurate health, demographic, and location information
- Use a secure and private device during consultations
- Follow your provider’s instructions, including lab testing or follow-up visits
- Report adverse reactions, errors, or concerns promptly
- Not use NavioMD services for emergency care or urgent psychiatric issues
LABORATORY TESTING & LIMITATIONS
If your provider orders a diagnostic lab test:
- Testing may be conducted via third-party laboratories not affiliated with NavioMD
- Lab results may be subject to variability or error (false positives/negatives)
- Providers rely on available information and lab data but are not responsible for test quality
- Lab results will be considered in the context of your full health profile
OPEN PAYMENTS NOTICE
Under the federal Physician Payments Sunshine Act, manufacturers of medical products must report payments made to healthcare providers. You may review these records at: https://openpaymentsdata.cms.gov
PRIVACY & DATA SECURITY
NavioMD and its partners comply with HIPAA and all relevant data privacy regulations. Your data is protected using industry-standard encryption, secure cloud infrastructure, and access controls.
We will not disclose your protected health information (PHI) except:
- As required by law
- For purposes of treatment, payment, or healthcare operations
- To fulfill your request for care via NavioMD or its partners
- When necessary to prevent harm or report abuse (per mandatory reporting requirements)

CONSENT, WITHDRAWAL & CHOICE
By accepting this Consent, you confirm:
- You voluntarily choose to receive telehealth services
- You understand the nature, risks, and limitations of care
- You are located in a jurisdiction where NavioMD is authorized to operate
- You may use any pharmacy of your choice and may request prescription transfers
- You agree to pay for all services and will not submit claims to Medicare, Medicaid, or other government insurers
- You may withdraw your consent at any time by emailing support@naviomd.com (note: this does not impact services already rendered)
